The USA is state based, not one national licence

National exams exist, but the actual licence is issued by a state board. That means the correct English test, credential report and application route can change by profession and state.

Doctors - ECFMG / Intealth pathway

For international medical graduates aiming for ECFMG Certification, ERAS, the NRMP Match, residency training and later state medical licensure. OET Medicine is part of the communication skills requirement for Pathways applicants.

English-test planning: Official ECFMG 2027 guidance requires OET Medicine for Pathways applicants, with 350 in Listening, Reading and Speaking and 300 in Writing in one test administration. This updates the older PDF score wording, so candidates must follow ECFMG current rules.

Nurses - State BON / NCLEX pathway

State Boards of NursingCGFNS/TruMeritNCLEX-RN

For internationally educated nurses applying state by state. NCSBN owns the NCLEX-RN, but each Board of Nursing decides application requirements, credential evaluation, English testing, fingerprinting, SSN policy and authorization to test.

English-test planning: Do not assume OET Nursing will count for US nursing. The PDF warns that TOEFL iBT or IELTS Academic is usually the relevant English route, depending on the state board and VisaScreen requirements.

If you also target the UK, OET Nursing supports NMC preparation, but it may not support your US BON route.

Listening

Prepare for US accents and workplace communication, but choose the English test your target authority accepts.

Pharmacists - NABP / FPGEC pathway

NABPFPGEC and FPGEETOEFL iBT

For foreign pharmacy graduates seeking FPGEC Certification before state board eligibility, NAPLEX, MPJE or a state law equivalent, internship hours and final licensure.

English-test planning: NABP/FPGEC requires TOEFL iBT as part of FPGEC Certification. Current NABP guidance says all four sections must be completed in one testing session, MyBest scores are not accepted, and remote-proctored/at-home TOEFL is not accepted.

Physical therapists - FSBPT / State PT pathway

English-test planning: The PDF positions TOEFL iBT as the standard US physical therapy English-test route. Do not book OET Physiotherapy for the US unless your exact state board explicitly confirms acceptance; OET Physiotherapy is primarily useful for UK HCPC and selected non-US bodies.

Do USA doctors need OET Medicine?

Yes, current ECFMG Pathways guidance requires OET Medicine for Pathways applicants and states there are no exceptions for native language, medical school language or citizenship.

What OET score does ECFMG currently require?

Current 2027 ECFMG guidance lists 350 in Listening, Reading and Speaking and 300 in Writing in one test administration. Candidates must verify the current ECFMG cycle before booking.

Can nurses use OET Nursing for the USA?

Do not assume that. US nursing is state-board dependent and commonly uses TOEFL iBT or IELTS plus credential evaluation and NCLEX-RN planning.

Is US licensure the same as a visa?

No. Licensure, certification, credentialing and visas are separate. Candidates should verify official licensing rules and seek licensed immigration advice for visa questions.
